Step 1: Transfusion-Related Acute Lung Injury (TRALI) is an acute respiratory distress syndrome that develops during or shortly after a blood transfusion. Step 2: By definition the onset is within 6 hours of transfusion, so the correct option is 6 hours. Step 3: It presents with rapid dyspnoea, tachypnoea, fever, cyanosis and hypotension, with bilateral non-cardiogenic pulmonary oedema on the chest X-ray (white-out) and no signs of circulatory overload or CHF.
