Step 1: The image is a hysterosalpingogram (HSG), where radio-opaque dye fills the uterine cavity and outlines both fallopian tubes.
Step 2: Both tubes are seen distended and ballooned at their distal (fimbrial) ends, and there is no peritoneal spillage of dye. This dilated, sausage-shaped distal tube is the classic look of hydrosalpinx caused by a fimbrial block.
Step 3: So the finding is bilateral hydrosalpinx. In a cornual block the dye stops at the uterine horn and the tubes do not fill at all. In a normal HSG the dye flows freely through patent tubes and spills into the peritoneal cavity. A bicornuate uterus shows two separate uterine horns. None of these match the distended fluid-filled tubes seen here.
