Answer: Duodenum.
Context: Pancreatic and duodenal neuroendocrine tumours occur in roughly half of MEN-1 patients. Among the functional ones, gastrinoma is the most common, and in MEN-1 these gastrinomas are typically small, multiple and located in the duodenum rather than the pancreas.
Why option A: The most frequent site of a MEN-1 associated gastrinoma is the duodenum (the so-called gastrinoma triangle, especially the first and second parts), followed by insulinoma, which is the next most common functional tumour and is pancreatic.
Why the others are wrong: Sporadic gastrinomas can be pancreatic, but in MEN-1 the duodenum is the dominant site, so pancreas (C) is not the best answer here. Jejunum (B) and ileum (D) are not typical gastrinoma sites.
