Step 1: The hysterosalpingogram shows a single, curved, elongated uterine cavity tapering toward one side (the banana-like configuration) with only one fallopian tube filling. This is a unicornuate uterus.
Step 2: A unicornuate uterus results from failure of normal development of one Mullerian duct, so only a single horn forms. On T2-weighted MRI it appears as a curved, elongated uterus with fundal tapering off the midline, best seen on axial oblique long-axis images.
Step 3: It is associated with increased spontaneous abortion, obstetric complications and ipsilateral renal abnormalities (because the mesonephric system is linked to Mullerian development).
Step 4: Why not the others - uterus didelphys has two separate cavities, cervices and often a vaginal septum; bicornuate uterus has two horns with a single cervix and a notched fundus; septate uterus has a single external fundal contour divided by a septum. The solitary curved banana-shaped cavity confirms unicornuate uterus.
