Concept:
• Urustambha (Stiffness of the thighs) is a unique disease where Ama (metabolic toxins) and excessive Medas (adipose tissue) along with Kapha obstruct the Vata in the lower limbs.
• Although it manifests as stiffness and paralysis (suggesting Vata), the underlying cause is highly "oily" and "heavy" (Abhishyandi).
Step 1: Understanding the pathology
In Urustambha, the Ama and Medas are Sneha-sadarsha (similar to oil/fat in properties).
They saturate the channels, making them heavy and immobile. This is described as a "cold and oily" obstruction.
Step 2: The effect of Snehana
Snehana (oleation therapy) involves the application of oils or fats.
Since the disease is already caused by an excess of oily and heavy substances (Ama/Medas), performing Snehana would be adding "fuel to the fire."
It would further increase the obstruction, making the thighs even stiffer and more difficult to treat.
Step 3: Determining the treatment line
Standard Vata treatments like Snehana and Basti (medicated enemas) are strictly contraindicated in Urustambha.
The treatment must always be Rukshana (drying treatments) and Kshapana (depletion of fat/Kapha) to clear the oily obstruction first.
