Answer: MCU with bulbar urethral stricture.
The study demonstrates a stricture (narrowing) in the bulbar segment of the anterior urethra.
Common causes of urethral stricture:
Inflammatory: after urethritis, or after balanitis xerotica obliterans (BXO).
Traumatic: bulbar urethral injury, and pelvic-fracture urethral disruption.
Iatrogenic: after urethral instrumentation or catheterization, transurethral or radical prostatectomy, and pelvic radiotherapy.
Idiopathic.
The bulbar urethra is the commonest anterior site for strictures, especially after straddle injury and inflammation. The narrowing here lies in the bulbar urethra, which makes the bulbar stricture the answer. Ref: Bailey & Love, Short Practice of Surgery, 27th ed, p. 1482
