Step 1: Cryoglobulinemia is the presence of large amounts of cryoglobulins in the blood. Cryoglobulins are immunoglobulin-based proteins that become insoluble and precipitate at reduced temperatures and redissolve on warming.
Step 2: Mixed cryoglobulinemia is made of mixtures of monoclonal or polyclonal IgM, IgG and/or IgA, together with complement proteins such as C4, and is strongly linked to chronic infection.
Step 3: The classic and commonest associated infection is Hepatitis C virus, which drives the great majority of mixed (type II and III) cryoglobulinemia cases. Hence Hepatitis C is the answer.
Step 4: Ovarian cancer, diabetes and leukemia are not the characteristic association; leukemia or lymphoma may rarely cause type I cryoglobulinemia, but the strongest and most tested link is Hepatitis C.
