Step 1: Bullous myringitis (also called myringitis bullosa or otitis externa haemorrhagica) is an acute, extremely painful, sudden-onset inflammation of the tympanic membrane.
Step 2: It is classically attributed to Mycoplasma pneumoniae infection, and may also follow a viral infection such as influenza.
Step 3: Characteristically, haemorrhagic bullae filled with serosanguinous fluid and blood appear on the tympanic membrane and deep meatus. A mild conductive deafness with scanty discharge may be present.
Step 4: On healing the bullae leave a Sago-grain appearance of the tympanic membrane. Pseudomonas, pneumococcus and Candida are not the typical cause, so the answer is Mycoplasma.
